PAPERWORK

Before your first visit we do require some information about you and your health. There are 3 possible ways you can accomplish this.

1. ON LINE: You may submit the registration and medical history information online. These forms will be automatically submitted to us after you complete the information online. This is the most popular method as it saves time on the day of your visit and allows you to make the most out of your consultation time with us.

2. DOWNLOAD You may download and print paper copies of the registration and medical history forms and complete them. You can either bring the completed forms with you on your visit or mail to us ahead of time. (attachments)

3. IN OFFICE You may use the traditional option of completing the registration and medical history information in our office on the day of your visit. If you choose this option please plan to arrive 15 minutes early to allow sufficient time for this.

CANCELLATION POLICY

We respect the value of your time and ask the same of you. Therefore, we request the courtesy of a 24 hour advanced notice for rescheduling or canceling your appointment. If this request is not honored, a $125.00 fee will be incurred and your credit card may be used to book future appointments. If it is a health appointment it will be the entire retail cost of the appointment All cancellation fees are donated to charity.

PRIVACY

The privacy of your personal and medical information will be fully maintained by our office.

FINANCING

We offer financing for procedures over $2000 through Lending USA.
